VOLUNTEER WAIVER

 We are grateful to you for volunteering for this event. We ask that ALL participants complete this waiver. If you are between 13 and 18 years old, a parent or legal guardian must agree to this waiver on your behalf and accompany you on this project, unless you are with a chaperoned, organization group with permission from United Way of Greater Charlotte to participate. Teen groups can only volunteer if there is a 1 to 1 ratio of teens to supervising adults.

 I understand that I am spending time as a volunteer on projects organized by United Way of Greater Charlotte with community organizations. I understand that, as a volunteer, I may be involved in activities that have a potential risk of injury. I agree that I will only perform volunteer activities that I am comfortable doing, I also agree that I will not hold United Way of Greater Charlotte or the community partner or other service recipient receiving the benefit of the project (collectively “the releasees”) responsible for any damage or injury to me or my property as a result of my participating in the project, including but not limited to any damage or injury arising from my participation in volunteer activities. I agree to be responsible for my behavior and indemnify and hold harmless the releasees from any damage or liability arising out of my activities as a volunteer in connection with United Way of Greater Charlotte. I will take responsibility for any personal equipment I bring to the project, and will only handle equipment I feel comfortable using.

 I grant full permission for organizers to use photographs, film or videotape of me or my likeness in legitimate accounts and promotions of this event.
